Our Registry covers the states of Alabama, Mississippi, Georgia, and Florida and is open to people of all ages from birth and up.

- Paper Registration. If your child is a minor, print the registration forms (Informed Consent, Background Questionnaire) and mail, email, or fax them back. Mail to UAIDPR, Attn Eric Cooks, Department of Psychology, Box 870348, The University of Alabama, Tuscaloosa, AL 35487-0348, email firstname.lastname@example.org, or fax back to (205) 348-8648.
- Telephone Registration. If your child is a minor, call the registry at (205) 348-4253 or (205) 887-4312, or we can call you. Telephone registration will take about 15 minutes, and may be done in the same phone call of Step 1.

Are you eligible?
1.) Do you or a family member have an intellectual disability or a syndrome closely related to intellectual disability (Ex: Down syndrome, Fetal Alcohol Syndrome, Fragile X Syndrome)?
2.) Are you willing to be contacted up to 3 times per year by registry staff about research studies?
3.) Are you willing to be willing to be contact 2 times per year by registry staff to update basic contact information?
If you answered yes to these 3 questions you or your family member are eligible for the registry!!
