How to Join
Our Registry covers the states of Alabama, Mississippi, Georgia, and Florida and is open to people of all ages from birth and up.
*Note: For adults with an intellectual disability looking to participate in the registry, please email us at idlab@ua.edu or call 205-348-4253 so that we may discuss your participation.*

If your child is a minor, print the registration forms (Informed Consent & Background Questionnaire), and submit them by mail, email, or fax:

Telephone Registration
If your child is a minor, leave a message at (205) 348-4253 or (205) 348-5083, and we will call you. Telephone registration will take about 15 minutes, and may be done in the same phone call of Step 1.
Eligibility
If you answer “yes” to these three questions, you or your family member are eligible for the registry:
- Do you or a family member have an intellectual disability or a syndrome closely related to intellectual disability (Ex: Down syndrome, Fetal Alcohol Syndrome, Fragile X Syndrome)?
- Are you willing to be contacted up to three (3) times per year by registry staff about research studies?
- Are you willing to be contacted two (2) times per year by registry staff to update basic contact information?
